Botox or filler in Lebanon: which one does what

Botulinum toxin relaxes movement lines; dermal filler replaces lost volume. How to tell which a line needs, onset and duration, and the risks in plain words — for patients considering treatment in Lebanon.

Two different jobs

Botulinum toxin and dermal filler are frequently mentioned together, but they do different jobs. Botulinum toxin is a muscle relaxant: it blocks the nerve signal to the muscle it is injected into, so the muscle cannot contract as strongly (ASPS). It treats lines caused by movement — frown lines between the brows, forehead lines, crow's feet at the corner of the eyes. Dermal filler is a volumiser: it replaces lost volume, restoring fullness to the cheeks, hollow areas under the eyes, and deep creases (AAD). It treats lines and hollows caused by lost volume, not by movement.

How to tell which a line needs

A line that only appears when the face moves — frowning, raising the brows, smiling — is a movement line, and softens with toxin. A line or hollow that is present even when the face is still is a volume loss, and needs filler to fill it. Some faces have both problems in different areas, and use both treatments, each where it is needed. This is assessed area by area at a facial assessment before any treatment is planned.

Onset and how long each lasts

Botulinum toxin does not work immediately: it begins working after three to five days, and is fully settled at ten days, which is when the result is reviewed. Filler results are immediate, visible the moment it is placed. Both are temporary, and the timelines differ by product and by area treated — full durations for toxin and filler are set out in how long Botox and filler last.

Reversibility

Hyaluronic acid filler can be softened or dissolved with an injected enzyme, hyaluronidase, if the result needs adjusting (JMIR Dermatology). Botulinum toxin has no reversal agent; its effect wears off on its own as the muscle regains function.

Who does it, and what comes first

Both treatments are medical procedures done with anatomy in mind, not cosmetic add-ons. At this practice, both are performed by Dr. Vincent ElKhoury, a board-certified plastic and reconstructive surgeon, at his clinic in Mansourieh, Metn. A consultation and facial assessment come first, area by area, before either treatment is planned.

Risks, in plain words

  • Bruising or bleeding at the injection site, with either treatment.
  • Asymmetry between the two sides of the face.
  • Lumps or under- or over-correction with filler.
  • A rare but serious vascular event with filler, where it enters a blood vessel and blocks circulation, which can damage skin or, near the eye, threaten vision (ASPS). Sudden severe pain, skin that changes colour, or a change in vision after filler needs urgent medical care.

Questions

FAQ

What is the difference between Botox and filler?

Botulinum toxin relaxes the muscles that create movement lines, such as frown lines, forehead lines and crow's feet. Dermal filler replaces lost volume, restoring fullness where it has been lost. They treat different problems, and the choice depends on whether a line comes from movement or from lost volume.

How do I know if I need toxin or filler?

A line that appears only when the face moves is a movement line and responds to toxin. A line or hollow present even when the face is still comes from lost volume and needs filler. Some faces need both, in different areas. This is assessed at a facial consultation before any treatment is planned.

Is dermal filler reversible?

Hyaluronic acid filler can be softened or dissolved with an injected enzyme called hyaluronidase if the result needs adjusting. Botulinum toxin has no reversal agent; its effect wears off on its own.

What are the risks of Botox and filler?

Both can cause bruising or bleeding at the injection site, and asymmetry. Filler can also cause lumps, under- or over-correction, and, rarely, a vascular event where filler enters a blood vessel and blocks circulation, which can damage skin or threaten vision near the eye. Sudden severe pain, skin colour change or vision change after filler needs urgent medical care.
