Two different problems
A stomach that does not flatten with diet and training has one of three causes: fat under the skin, loose skin, or abdominal muscles that have separated. Liposuction treats the first. A tummy tuck treats the second and the third. Choosing between them starts with naming which one is there.
Fat under skin that still springs back: liposuction
Liposuction removes fat through incisions of three to four millimetres, hidden in a crease or the navel. It changes the outline, not the number on the scales. It works when the skin is still elastic enough to shrink onto the new contour.
It does not tighten loose skin. It does not remove cellulite or stretch marks (NHS). Where the skin has lost its recoil, removing the fat underneath can leave it looser than before.
Loose skin and a separated muscle: tummy tuck
After pregnancy or a large weight loss, the skin below the navel can hang in a fold, and the two vertical muscles of the abdomen can stay apart. A tummy tuck removes that skin and stitches the muscle closed from the ribs to the pubis. No amount of core training closes a true separation.
The price is a permanent scar, low across the abdomen, placed to sit inside underwear. The navel is brought out through a new opening. Decide about the scar before deciding about the shape.
The recovery is not the same
- Liposuction: home the same day. The compression garment is worn day and night and stopped at four to six weeks. Gentle exercise at 3 to 4 weeks, strenuous activity after 10 to 12 weeks (NHS). The final outline shows at 3 to 6 months.
- Tummy tuck: one or two nights in hospital. Three to four weeks before desk work is comfortable, six weeks before real exercise, and a support garment for 6 weeks (NHS). The scar keeps fading for a year.

When both are done together
Liposuction of the flanks and waist can be added to a tummy tuck in the same operation, to give a better outline than the tummy tuck alone. It is planned as a single operation, not two.
What neither one is
- A weight-loss operation. Both are done at a stable weight. A large weight gain or loss afterwards changes the result.
- A way to skip pregnancy planning. A later pregnancy separates the repaired muscle again, so a tummy tuck is best done when the family is complete.
A quick way to tell
- A pocket of fat on the abdomen or flanks, with firm skin over it → liposuction.
- Skin that folds over below the navel, a bulge that stays after pregnancy → tummy tuck.
- Both at once → a tummy tuck with liposuction of the flanks.
